Now Accepting Applications — Healthy Opportunities Pilot (HOP) Food Box Delivery

Medicaid-enrolled Brunswick County residents may qualify for FREE healthy food box delivery through UCAN's Healthy Opportunities Pilot program, delivered in partnership with Community Care of the Lower Cape Fear (CCLCF).

Delivery Areas:

  • Bolivia
  • Supply
  • Shallotte
  • Calabash / Thomasboro
  • Ash
  • Longwood

Capacity: Up to 150 participants per week.

Important: This is a Medicaid benefit — there is no cost to eligible participants.

Simple Eligibility

  • Must be Medicaid-enrolled
  • Must live within the Brunswick County delivery area listed above

NCCARE360

UCAN is an active NCCARE360 community partner. Case managers, providers, and community organizations across the state can send referrals to UCAN directly through the NCCARE360 platform.

No Wrong Door

UCAN follows a "No Wrong Door" referral approach — meaning wherever a client first reaches out, whether directly to UCAN or through a partner organization, we help connect them to the right services, including Healthy Opportunities Pilot eligibility screening and other community resources. No one is turned away without direction to the support they need.
